Forms & leads

OmniTrustDXP treats form submissions as revenue events, not spreadsheet rows. Everything collected is a lead — and every lead is routable, scored, and measurable.

Building a form

  • Add a Contact Form or Appointment Booker block to any page.
  • Define fields inline (name, email, dropdowns, files) or link to a reusable Form record for cross-page reuse.
  • Set a success message, SLA target (in hours), and where to send an alert.
  • Turn on geolocation capture to enrich submissions with country and nearest branch.

Lead pipeline

When Generate lead is enabled on a form, every submission also creates a Lead record with status New. The lead moves through:

New → Contacted → Qualified → Proposal → Won or Lost

Leads are scored automatically based on source, product interest, and completeness — giving you a ranked follow-up queue.

Assignment rules

  • Round-robin within a lead team.
  • Rule-based — route by country, product interest, campaign source, or form department.
  • SLA escalation — if the first response breaches the SLA, escalate to a manager automatically.

Campaigns

Tag inbound leads with UTM campaigns and compare cost-per-lead, qualification rate, and revenue per campaign on the built-in analytics dashboard.

Webhooks & CRM

Every form can forward submissions to an external webhook (Zapier, HubSpot, custom CRM). The platform verifies destination IPs to prevent SSRF, retries on failure, and logs every delivery attempt.
